- Business registration application fee of $100.00 for each registration

- Category I facilities must reheat previously cooked and cooled potentially hazardous foods
- Category III facilities may only serve non-alcoholic beverages
- Category III facilities limited to preparation of non-potentially hazardous foods and beverages like snack foods and carbonated beverages
- Category I facilities with complex food preparation or extensive raw ingredient handling require hand contact for ready-to-eat foods
- Category I facilities must prepare potentially hazardous foods for off-premises service with time/temperature requirements
- Category I facilities require cooling of potentially hazardous foods as part of food handling operations
- Category I facilities serving immunocompromised individuals where they comprise majority of consuming population
- Category I facilities performing vacuum packing or reduced oxygen packaging at retail level
Hours of operation2 rules ▾
- Category I facilities cannot hold hot or cold potentially hazardous food for more than 12 hours before serving
- Category II facilities cannot maintain hot or cold foods at temperature for more than 12 hours and must restrict to same day service

- Corporate owners must provide corporate name, Federal ID, and office address

- Category III facilities may only serve pre-packaged foods from approved food processing plants
- Category II facilities must obtain foods served at establishment requiring complex preparation from approved food processing plants or Category I establishments
