- Cannot operate a hotel, restaurant, temporary restaurant, tourist rooming house, vending machine commissary or vending machine without annual license
Commissary1 rule ▾
- All food must be prepared in temporary food stand or licensed facility/base, no home preparation allowed
Penalties4 rules ▾
- Personal information misuse may be subject to penalty under Wis. Stat. § 15.04(1)(m)
- No license may be issued until all applicable fees have been paid
- Operating without a license is a violation of Wisconsin law
- Operators without license must pay surcharge in addition to license fee
Other rules10 rules ▾
- Department has 30 days to approve/deny license application after receiving complete application
- License shall not be issued to operator without prior inspection
- Department must provide written reasons if license application is denied
- Complexity levels: Prepackaged (0), Simple (0.25 or less), Moderate (2.75-4.5), Complex (4.5 or more)
- At least two handwashing sinks or stations are required, conveniently located near food activity stations and dish cleaning areas, cannot be blocked by doors or equipment, and all faucets must be hands-free
- Floor, wall and ceiling finishes must be smooth, non-absorbent, and light colored, with specific approved materials including fiberglass reinforced plastic panels, high gloss enamel paint, commercial vinyl floor tile, and vinyl coated drop-in acoustical tile
- At least 1 service sink or 1 curbed cleaning facility with floor drain is required, conveniently located for cleaning mops or similar wet floor cleaning tools and for disposal of mop water and similar liquid waste
- Residential coolers are not allowed and residential freezers are allowed only for storage of commercially frozen foods
- Facility needs adequate capacity to store soiled utensils prior to washing and sufficient space for air drying of clean utensils, with equipment available to adequately wash, rinse and sanitize dishes and utensils
- A food prep sink is required when food items need cleaning or thawing by placing them in a sink below the flood rim
