- Not licensed to operate until Department conducts inspection
- Department may inspect premises at any reasonable time
- Licenses are not transferable between persons or locations
- License shall not be issued to operator without prior inspection

Eligibility1 rule ▾
- No person may conduct, maintain, manage or operate a hotel, restaurant, temporary restaurant, tourist rooming house, bed and breakfast, vending machine commissary or vending machine if the person has not been issued an annual license
Commissary8 rules ▾
- All food must be prepared in temporary food stand or licensed facility/base, no home prepared foods allowed
- Residential freezers are allowed only for storage of commercially frozen foods
- At least 1 handwashing sink or station is required near food activity stations and dish cleaning areas
- At least 1 service sink or 1 curbed cleaning facility with floor drain is required for mops and cleaning tools
- Food prep sink is required when food items need cleaning or thawing by placing them below the flood rim
- Facility needs adequate capacity to store soiled utensils and sufficient space for air drying clean utensils
- Residential coolers are not allowed
- Floor and finish materials must be fiberglass reinforced plastic panels, high gloss enamel paint, commercial vinyl floor tile, or vinyl coated drop-in acoustical tile

- No license may be issued until all applicable fees have been paid
- If application denied, department shall give written reasons for denial
- Operating without a license is a violation of Wisconsin law
- If operating without license, must pay surcharge in addition to license fee
